Facial feminization surgery (FFS) is focused on reshaping your face to reflect more feminine proportions and features. From forehead recontouring and jaw reduction to rhinoplasty and lip lifts, FFS includes a wide range of surgical and non-surgical procedures. At Rivé Plastic Surgery, we tailor each to your individual anatomy and goals using our experience and precision to guide you through every step of your transformation.
Facial feminization surgery is a collection of surgical and non-surgical procedures designed to reshape and soften masculine facial features into a more feminine appearance. You usually find FFS focusing on areas including your forehead, brow, nose, cheeks, lips, jaw, and chin. These areas are where bone structure and soft tissue most visibly differ between male and female faces.
FFS is sought by transgender women and nonbinary individuals as part of their gender-affirming care, as well as by cisgender women who want to soften strong or angular features. Whatever your reason for pursuing FFS, our goal remains the same: to help you discover a face that feels authentically yours.
The cost of FFS depends on which procedures are included in your treatment plan. Individual procedures can range from around $1,500 for non-surgical treatments like Botox to $10,000 or more for surgical procedures such as forehead recontouring. A comprehensive FFS plan combining multiple procedures can range from $20,000 to $50,000 or more depending on scope.

You’ll likely be an ideal candidate for facial feminization surgery if you:
During your consultation, your surgeon will review your medical history, discuss your goals, and recommend a personalized combination of procedures best suited to help you achieve those goals.
Forehead recontouring and brow lift reduces a prominent brow ridge and raises your hairline or brow position for a softer, more open upper face.
Rhinoplasty reshapes your nose to create a narrower bridge, refined tip, or more upturned angle characteristic of feminine facial proportions.
Cheek augmentation enhances cheek volume and projection using implants or fat transfer for a rounder, more feminine midface.
Jaw contouring reduces the width and squareness of your jaw through bone reshaping, creating a softer, more tapered lower face.
Chin reshaping narrows and rounds your chin to feminize the lower third of your face.
A lip lift shortens the distance between your nose and upper lip, increasing lip visibility and creating a more feminine-looking mouth.
For patients seeking subtle refinement or who are not yet ready for surgery, we offer non-surgical options that can feminize your face with little to no downtime including dermal fillers, BOTOX, Dysport, Renuva, and Kybella.

Every FFS journey is unique and your surgeon will be upfront about everything during your consultation. At your first appointment, our team will use imaging tools to help you visualize potential outcomes based on your specific anatomy and goals.
Results vary depending on the procedures performed, but our patients usually notice significant softening of masculine features, more facial harmony, and a face that better reflects who they are. Our entire team is committed to your comfort and will explain everything about what to expect during appointments leading up to your procedure.
Recovery from facial feminization surgery varies depending on which procedures were performed. While timelines do vary, most patients generally can expect the following post-surgery.
Weeks one and two, you’ll experience swelling, bruising, and discomfort. These are normal and expected but you will need to rest at home during this period. Elevating your head, using cold compresses, and taking prescribed medications will help manage discomfort. As you approach weeks two to four, visible bruising usually fades significantly, although swelling may persist.
By months one through three, most of your visible swelling will face and you’ll gradually return to normal activity. Final results are usually fully visible by six to twelve months as tissues settle and residual swelling completely clears.
Coverage for FFS varies significantly by insurance provider and plan, however, our office does not bill insurance.
There is no single standard FFS procedure. Your surgery will be customized based on your anatomy, goals, and conversation with your surgeon. Possible procedures include forehead recontouring, rhinoplasty, cheek augmentation, jaw and chin reshaping, lip lift, tracheal shave, and more. Your surgeon will outline exactly what is included in your personalized treatment plan during your consultation.
FFS is a subset of gender-affirming procedures focused on modifying masculine facial features to create a more traditionally feminine appearance. While Gender Confirmation Surgery (GCS) is an umbrella term that includes FFS, it is more commonly referred to when discussing genital reconstruction surgery.
Our team will work with you to assess your facial structure, listen to your aesthetic goals, and recommend the procedures most likely to achieve the feminine facial appearance you’re looking for. There is no one-size-fits-all approach.
This depends entirely on the number and complexity of procedures being performed. A single procedure such as a tracheal shave may take under an hour, while a comprehensive FFS surgery combining multiple procedures may take six to eight hours. Your surgeon will give you an estimated duration as part of your surgical plan.
